2) You will be prompted to ENTER your personal intake information, andname of any minors that may use your account during your physical presence,via our secure server. In addition you will be prompted to sign the Patient registration forms and release forms. You will have a chance to PRINT OUT the HIPAA Notice of Privacy Practices form for your records. Here you will also get a chance to complete a REVIEW OF SYSTEMS AND HISTORY FORM. All records will be kept confidential as per HIPAA guidelines.

3) If you will require a NEW or REFILL on any MEDICATIONS you will be asked to FAX us a copy of a Valid Photo Identification, such as Driver’s License or other State or Government issued photo identification card to 1-888-9COOLMD or 1-888-926-6563. This information will remain confidential as part of your medical records. This measure will be useful to prevent fraudulent use of our services and prevent identity theft.

5) Upon completion of your Medical consultation you will have a Script Faxed or called normally within 5-10 minutes post-consult to a pharmacy of your choice. All scripts are written at the discretion of the evaluation physician or health care provider. Other arrangements may involve calling in your script to a mail order pharmacy to have the medications sent directly to your home address. We may require additional information from you such as any prescription card programs that you are use, insurances used, or other information useful in helping us complete the calling in of the script.
